Insurance and Billing Information

If you have dental insurance you must realize that your insurance is a contract between you, your employer, and the insurance company. We are not a party to that contract (Out of Net-work). Our staff will be happy to discuss all insurance details with you at your appointment.

Consultation fees are to be paid at time of visit, we do not accept your insurance for this visit but we will submit to your insurance for re-imbursement for you. If treatment is needed your insurance will be contacted to verify your benefits. Please note we may not be able to contact your insurance immediately but we will try our best to provide you with your insurance information as soon as we are able to. If we need to submit any Pre- Estimates, most insurances will respond within four to six weeks. Any remaining balance after your insurance has paid is your responsibility. Your prompt remittance is appreciated.

For your convenience we accept Visa, Mastercard, and Discover. We do not accept checks. If you have any problems or questions, please ask our staff.
